This is an AliMed model 932468 Freedom OR systems Rhinoboard hourglass arm board for an operating room table.  This has a "universal design" and can be used with any operating room table - clamps are required.  Measurements are 33" long and 23" wide at widest point, and 15" wide at narrowest point.
